Abstract
Over the course of decades, a national movement to promote advance care planning (ACP) has taken
shape and now every state and the District of Columbia has endorsed paradigms for physician orders for life-
sustaining treatment (POLST). These laws have established mechanisms for clinicians to work with patients
and their healthcare proxies to document people’s wishes for medical care. Despite the widespread
endorsement of this approach, many older patients with serious illness have still never documented their
medical wishes. This is particularly important for people with Alzheimer’s disease and related dementias
(ADRD) as clinicians frequently miss the opportunity to engage in such planning while people retain decisional
capacity. Indeed, most people with ADRD do not have documented advance care plans. While improving
documentation of wishes is important, the goal of this work is to help ensure that patients receive care that is
concordant with their wishes. Much improvement is needed. Over 90% of people express a desire for some
forms of limitation to medical care, yet people’s goals are frequently not realized.
For more than a decade, our research team has worked to inform and empower patients and their care
partners (e.g., family member) to document and attain their advance care plans. Our research has focused on
brief video decision aids that patients and their care partners watch with health providers as a trigger to discuss
patients’ goals of care. These tools are now in 30 different languages. The Advancing Ethical Goals In Society
(AEGIS) Trial of our ACP Counselor Video Program has been designed to address the limitations of prior ACP
work. The AEGIS Administrative Core will be responsible for day-to-day management and functioning of the
research. Its goal is to ensure that the scientific and programmatic activities of the AEGIS Trial and associated
Implementation Core (IC) and Ethics Core (EC) are carried out with high quality and timeliness.
The Administrative Core will: (1) Provide leadership and organizational structure for a collaborative,
integrated approach to the research goals; (2) Guide all aspects of data collection, data management, data
quality, and analyses; and, (3) Encourage the sharing of data with investigators, and wide dissemination of the
trial results to healthcare systems. The AEGIS Team will conduct and disseminate interdisciplinary research
aimed at improving ACP and attaining goal-concordant care for older patients with ADRD or other serious
illness. With the Administrative Core’s scientific and administrative leadership, we will capitalize on the massive
test bed provided by our outstanding clinical collaborators as well as educational, ethical, research, data
processing, analytic, and management capabilities of the AEGIS Team to undertake a significant trial to
advance the science of ACP.document fetched 2026-07-12 08:14
